From CCSD - Effective July 1, 2022, students enrolled in Grade 12 throughout Nevada are required to receive the meningococcal vaccine prior to the start of the 2022–2023 school year. The meningococcal vaccine is an immunization against meningitis. The requirement, approved by the State Board of Health as a State Regulation, is for all public, private, and charter schools in Nevada. Students may not be permitted to attend school without this vaccination documentation or approved exemption.

The meningococcal vaccine is an immunization against meningitis. This requirement, approved by the State Board of Health as a State Regulation, is for all public, private, and charter schools in Nevada. As your student will be in Grade 12 for the upcoming school year, please provide proof of the required vaccination for your student to the school’s registrar or school health office prior to the first day of school. Students may not be permitted to attend school without this vaccination documentation or approved exemption. Vaccination Exemptions
All exemptions of student immunizations for religious beliefs must be submitted on the State of Nevada Preschool-12th Grade – Religious Immunization Exemption Certificate. This form must be completed and submitted annually to school offices.
